These nine structured profiles are deliberately selected anchors for recurring digital-safety mechanisms. They are not the complete evidence base and should not be used to infer frequency or trend.

Each profile separates the coronial concern, any respondent account, reviewer coding, limitations and Defence applicability. Reviewer codes and candidate Defence requirements are analytical layers; they are not attributed to the coroner.

Evidence status

All nine primary Judiciary reports were verified on 14 July 2026. Official responses are used where listed and remain explicitly distinct from coronial findings. Current product behaviour, wider prevalence and Defence-system applicability require separate evidence.
