Thematic analysis

This section applies explicit analytical lenses to the canonical Regulation 28 evidence. It is an output, not a second findings register: case facts remain in the evidence base, recurrent mechanisms in Findings, and controlled Defence translations in the REQ- and AST- registers.

Current outline

• Clinical and patient safety: Regulation 28 evidence and DMICP — a two-page outline that distinguishes patient safety from DCB0129/DCB0160 clinical safety, maps the 71-report corpus to lifecycle controls, and cross-references the result with verified and unverified DMICP context.

The outline is a hazard-discovery and assurance-planning aid. It is not a DCB conformity assessment, a finding that DMICP contains a particular defect, or an assertion that a coroner has made findings about Defence.
